About The Position

This is a part-time Hospice Triage RN position within the Supportive Care Team (SCT) of the Divisional Nursing department. The role involves collecting and analyzing patient data from telephonic contacts and EMRs to determine issues and available resources for resolution. The RN will prioritize situations based on criticality, patient welfare, safety, and service impact. They will identify and address barriers to resolution, develop and communicate care plans, implement identified plans, coordinate care delivery, and evaluate progress towards outcomes. The position requires practicing ethically, delegating care appropriately with minimal supervision, promoting shared governance, and contributing to quality nursing practice through research and evidence-based practice. Effective communication and leadership in the professional practice setting are also essential.

Requirements

  • Graduate from an accredited School of Nursing.
  • Current Basic Life Support for Healthcare Provider status according to American Heart Association.
  • Current RN license or temporary license as a Registered Nurse Petitioner in the state in which you work and reside or; if declaring a National License Compact (NLC) state as your primary state of residency, meet the licensure requirements in your home state; or for Non-National License Compact states, current RN license or temporary license as a Registered Nurse Petitioner required in the state where the RN works.
  • Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling.
  • Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies.
  • Must speak English in good, understandable terms.
  • Intact sense of sight, hearing, smell, touch.
  • Finger dexterity.
  • Critical thinking and ability to concentrate.
  • Must be able to respond quickly to changes in patient conditions.
  • Physical abilities testing required.

Nice To Haves

  • BSN preferred.
  • Additional education, training, certifications, or experience may be required within the department by the nurse leader.

Responsibilities

  • Collects data pertinent to the healthcare consumer's health or the situation from telephonic contacts and/or related EMR information available.
  • Analyzes the data to determine the issues and available resources to seek effective resolution.
  • Prioritizes multi-issue, situations appropriate to the criticality of patient health and welfare, patient/staff safety and impact to effective, timely services.
  • Identifies barriers to situation/complaint resolution (i.e. language, literacy, psychological, etc) and follows-up with the appropriate resources or leadership to address timely and effectively.
  • Develops a plan that is effectively communicated with the parties involved and supports quality patient outcomes and patient satisfaction.
  • Implements the identified plan, coordinates care delivery, and employs strategies to promote health and a safe outcomes.
  • Evaluates progress toward attainment of outcomes.
  • Practices ethically.
  • With minimal supervision, delegates elements of care to appropriate healthcare workers in accordance with any applicable legal or policy parameters or principles.
  • Promotes shared governance and decision making activities, developing and nurturing research to positively affect clinical outcomes and promotion.
  • Attains knowledge and competence that reflects current nursing practice.
  • Integrates evidence and research findings into practice.
  • Contributes to quality nursing practice.
  • Communicates effectively in a variety of formats in all areas of practice.
  • Demonstrates leadership in the professional practice setting and the profession.
