About The Position

Utilizes the nursing process for assessment, planning, implementation, and evaluation to provide safe, therapeutic care. Functions as a member of the interdisciplinary and nursing teams; carries out established nursing procedures and performs related duties as required; performs nursing care for patients with more complex needs with assistance and supervision. Assists with the management of patient care administered by other members of the nursing team. Under general supervision, guided by the policies and procedures of the Medical Center and Ambulatory Services, and in collaboration with the physician staff and administrative management, the Pediatric Neurology Nurse works independently to coordinate the clinical care for Pediatric Brain Center clinic patients. The Pediatric Neurology Nurse will utilize a high level of clinical knowledge to screen patient referrals, coordinate patient evaluations to ensure efficient patient care flow, provide patient education, and improve health literacy and adherence to the treatment plan. The nurse serves as a clinical liaison between families, providers, inpatient teams, and external partners to ensure continuity of care across settings.

Requirements

  • Minimum of three (3) years of RN clinical employment experience in pediatric neurology and care coordination experience either in ambulatory practice or case management within the past 3 years.
  • Proven ability to work effectively with multidisciplinary healthcare team.
  • Excellent communication skills (written & verbal) and professional conduct.
  • Computer literate (esp. MS Word, Outlook, and internet use) and experience with EHRs
  • Efficient In Basket and Pool management skills to support timely, safe, and effective care delivery.
  • High level organizational skills, proficiency in meeting deadlines, ability to work independently and prioritize workload.
  • Demonstrated clinical competence including effective leadership, teaching, and communication skills or an equivalent combination of education and experience.
  • This position requires flexibility to orient and work at all UCSF Medical Center locations.
  • Current licensure as a Registered Nurse in the State of California.
  • Current healthcare provider CPR certification

Responsibilities

  • Responsible for maintaining the knowledge and abilities essential to the performance of stated behaviors.
  • Carries out legal responsibilities within the scope of the Nurse Practice Act.
  • Accountable to the Administrative Nurse Manager for overall work performance.
  • Provides individualized nursing care that responds to the growth and developmental needs of patients and demonstrates an understanding of the range of treatment required by the patient’s developmental level.
  • Advocates for patients/families by ensuring their needs and preferences are considered during treatment, enhances continuity of care, and assists patients in navigating the healthcare system.
  • Customizes patient care plans based on patients’ health conditions, diagnoses, and unique requirements and reports relevant changes in physical and emotional conditions to the healthcare team.
  • Responds routinely to patient calls including rotating coverage of the nurse advice line, and MyChart messages and provides appropriate telephone triage within scope of practice.
  • Identifies appropriate resources (e.g. social services) for patients with social, parenting, or personal concerns.
  • Recognizes and reports specific physical and emotional conditions/needs/concerns of the patient to appropriate health care team members via staff messages and/or documentation of the Encounter in ApeX (EPIC).
  • Provides in-clinic support including but not limited to patient rooming, medication preparation, procedure set up, maintaining Pyxis access, administering medications, oxygen therapy, specimen collection for genetic testing, and assists in emergency response.
  • Completes patient and caregiver forms and letters based on school and employment requirements within scope of practice and uses PBC Standardized forms to the extent possible.
  • Identifies relevant resources for patients with social or personal concerns.
  • Coordinates and attends multidisciplinary clinics.
  • Processes medication refills and obtains prior authorizations per policy and departmental guidelines.
  • Coordinates laboratory requisitions and instructs patient/family on how and when to obtain laboratory services.
  • Assist the practice coordinator as appropriate in obtaining outside medical records, imaging, and biopsies etc.
  • Provides vacation or sick call coverage for other groups within PBC based on skills/training.
