About The Position

This role focuses on providing a continuum of care for patients, ensuring seamless transitions and minimizing delays. As a liaison, the Registered Nurse Patient Navigator will foster stronger collaboration and promote patient-centered care. The position involves treatment coordination to achieve better patient outcomes and satisfaction, as well as data tracking for evidence-based improvements and efficiency.

Requirements

• Licensed Registered Nurse obtained prior to hire date or job transfer date required.
• Registered Nurse credentialed from the Oklahoma Board of Nursing required.
• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
• 3+ years RN experience.
• Excellent organizational, communication and prioritization skills

Nice To Haves

• BLS Provider preferred.
• Critical Care experience a plus
• BSN or higher preferred
