We are currently hiring for a Patient Navigator at our Plantation facility. This will be a full time position. The Women's Health Patient Care Navigator serves as the primary point of contact for patients requiring follow-up after a screening mammogram or other breast imaging. A key member of our women’s health program, you'll help ensure every patient receives timely, compassionate, and coordinated care from abnormal screening results through diagnostic resolution.

Requirements

  • High School Diploma or equivalent experience.
  • Minimum 2 years of experience in a healthcare setting, preferably in breast health or women’s health
  • Strong verbal and written communication skills with the ability to engage patients and physician offices professionally
  • Excellent organizational skills with demonstrated ability to manage and prioritize a dynamic, high-volume workload

Nice To Haves

  • Associate's Degree or Equivalent Experience
  • Bilingual (English/Spanish)
  • Experience with RIS, EMR, or practice management systems
  • Knowledge of MQSA lay letter requirements and/or breast imaging compliance standards

Responsibilities

  • Manage a daily worklist of BI-RADS 0 patients to ensure timely outreach and scheduling of diagnostic imaging; includes contacting patients to communicate results, explain next steps, and schedule recommended follow-up appointments as well as contacting referrers to obtain orders and/or missing priors.
  • Support specialized pathway enrollment, including calling referring physician to obtain an order and engaging the patient to schedule (e.g., BRCA mutations or strong family history indicating need for annual breast MRI; dense breasts indicating need for annual ultrasound; BIRADS 3 indicating need for 6-month follow-up)
  • Build trust and confidence with patients by explaining next steps clearly and compassionately.
  • Maintain accurate and up-to-date documentation in our breast navigation tracking system, including follow-up status, patient contact attempts, and appointment outcomes.
  • Track and report on key performance indicators including recall rates, diagnostic conversion, and follow-up completion rates.
  • Support the broader women’s health program by identifying process gaps and providing feedback to improve workflows and patient experience.
  • Cover an estimated 3-5 imaging locations, though coverage could shift based on patient workload.
