About The Position
This role focuses on coordinating care and improving patient outcomes within the Population Health department at Ascension Saint Thomas in Nashville, TN. The Registered Nurse Patient Navigator will act as a liaison between various stakeholders, advocate for quality improvement and education, optimize data and processes, and provide direct navigation support to uninsured and underinsured patients.
Requirements
- Licensed Registered Nurse credentialed from the Tennessee Board of Nursing OR current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
Responsibilities
- Build strong, collaborative relationships between clinical teams, administration, physicians, managed care partners, and community organizations to streamline care delivery.
- Coordinate comprehensive treatment plans while driving clinical quality improvement initiatives and staff educational programs.
- Track outcomes, compile critical performance data, and deliver actionable recommendations for continuous operational and clinical improvements.
- Connect uninsured and underinsured patients with vital community resources, follow-up care, and financial assistance programs.
