About The Position

This role serves as the primary liaison between administration, physicians, and managed care partners. The Registered Nurse Patient Navigator will direct treatment efforts while championing quality improvement and staff education. This position involves compiling metrics and tracking outcomes to drive high-level process improvements, managing specialized referrals and follow-up care for uninsured and under-insured populations, and fostering vital relationships with community organizations to extend the continuum of care.

Requirements

  • Licensed Registered Nurse credentialed from the Texas Board of Nursing required.
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

  • BLS Provider preferred. American Heart Association or American Red Cross accepted.
