About The Position

Vanderbilt Health is seeking a Registered Nurse for Palliative Care, full-time night shift at VUH. This role involves applying the nursing process to plan and implement patient care, including education and continuum of care planning. The nurse will collaborate with the healthcare team, establish patient goals, and evaluate progress. Palliative Care focuses on adult patients (18+) with life-ending conditions, emphasizing symptom management, life goals, and respecting patient/family wishes and culture. The shift is 7:00 PM - 7:00 AM, three times per week, totaling 36 hours, including some weekends.

Relevant Work Experience
Graduate of an approved discipline specific program
1 year of experience

Responsibilities

Applies the nursing process to plan and implement the care of patients, including patient and family education and continuum of care planning.
Effectively communicates pertinent patient/family information to the health care team.
Collaborates in establishing patient goals and evaluates progress to ensure effective outcomes.
Collaborates in establishing the plan of care, coordinates and implements care delivery while anticipating patient care needs across the continuum.
Analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis.
Continually utilizes the nursing process to evaluate patient's progress towards goals and applies appropriate interventions to ensure effective outcomes.
Employs strategies to promote health and a safe and healthy environment.
