About The Position

Frontier Behavioral Health (FBH) is a non-profit organization with over 100 years of service in Spokane, dedicated to providing evidence-based treatment for adults, youth, and seniors facing behavioral health challenges. As a Trauma-Informed Care Organization, FBH believes in recovery and empowering clients to lead fulfilling lives. The Program for Assertive Community Treatment (PACT) is an intensive, integrated community mental health service delivery model. The multidisciplinary PACT team shares a caseload of individuals with severe mental illnesses, often co-occurring with substance abuse, trauma, and other significant challenges. The team nurses, in collaboration with psychiatric care providers, manage and administer medications, monitor clients for medical issues and side effects, coordinate with other medical providers, and engage in health promotion and education. PACT operates 7 days a week from 8 a.m. to 5 p.m., with rotating weekend shifts to ensure client coverage.

Requirements

  • RN licensed in the State of Washington.
  • Active BLS within 90 days of hire and must maintain active BLS.
  • Must have an unrestricted and valid driver’s license.
  • Must be able to pass a motor vehicle screening and driving test.
  • Must have active car insurance.
  • Willing to use their personal vehicle in the course of work.
  • Knowledge of Washington Administrative Codes and statues pertaining to the treatment and delivery of mental health services, or ability to obtain within 3 months of employment.
  • Knowledge of service delivery models including groups, skills training brief/focused therapy, community-based interventions for adults with serious and persistent mental illness, or ability to obtain within 3 months of employment.
  • Ability to communicate and engage effectively with individuals of various backgrounds, trauma experiences, cultures, education level, and socio-economic status.
  • Excellent communication skills, including the ability to send and receive oral and written messages, in a clear, positive, prompt, and appropriate manner, while maintaining customer confidentiality.
  • Ability to drive a vehicle, traverse distances up to several city blocks and on various terrains, navigate stairs, get in and out of vehicles frequently is required.
  • Daily use of a computer is required.

Responsibilities

  • Provide symptom and medication assessment, evaluation, education, and management to individuals with persistent mental illness.
  • Consult with Psychiatrist, MHCP, hospital, community providers, client, and client’s family/guardian for care.
  • Process and sign medication orders, complete medication refills and prior authorization for specific medications based on provider request.
  • Administer medications to consumers as directed by FBH Psychiatrist.
  • Ensure access to medications through timely refills, by obtaining sample medication per order and by appropriate use of FBH discretionary funds or patient assistance programs for medications.
  • Ensure pertinent medication-related information is conveyed to hospital staff in the event of client hospitalization.
  • Participate in rotating weekend and holiday shifts as needed.
  • Assist in the development and delivery of staff and/or consumer education in the form of group classes or trainings.
  • Identify and include the client’s support network and applicable involved community partners in care coordination.
  • Develop and implement care plans, measurable objectives, and obtainable goals, in collaboration with the client, evaluating client progress and adjusting as needed.
  • Assist clients in identifying and accessing resources and collaborate with community resources on behalf of clients.
  • Assist clients in the development of healthy coping mechanisms in individual or group settings.
  • Maintain client records and all other required documentation in a timely and confidential manner in accordance with agency policies and procedures.
  • Conduct risk assessments, develop and implement safety plans, and provide crisis intervention and stabilization services as needed, including facilitating access to inpatient facilities for behavioral health care.
  • Represent the agency and its services to the community in a positive and professional manner.
  • Participate in required staff meetings, including regular supervision and daily clinical staff meetings.
  • Participate in any required training, consultation calls, and onsite visits or review when required by contract.
  • Accept delegation and follow through with specific expanded assignments and responsibilities within FBH system such as quality improvement teams, task groups, committees, consultations, mentoring or training responsibilities.
  • Establish and maintain collaborative working relationships with; clinical, administrative, support staff, community partners, clients, and family/natural supports.
  • Demonstrate teamwork and goal-oriented productivity.
  • Perform FBH charting requirements, correspondence, Workday and other paperwork in a complete and timely manner.
  • Operate as a member of a multi-disciplinary team and ensure services are provided in the community and clinical setting as required by contract.
  • Requires travel to see clients in the community and travel between work sites either via FBH owned Vehicles and/or personal vehicle.
  • Other duties as assigned.
