Forensic Assertive Community Treatment (FACT) Registered Nurse (4175)
About The Position
Carolina Outreach is seeking a Forensic Assertive Community Treatment (FACT) Registered Nurse to deliver medical and psychiatric care to individuals with serious mental illness and co-occurring substance use disorders who are involved in the criminal justice system. As a Forensic ACT RN, you will provide medication management, health monitoring, crisis intervention, and care coordination while working closely with law enforcement, courts, probation/parole, and correctional healthcare providers to ensure continuity of care and successful community reintegration. This role emphasizes forensic treatment planning, risk assessment, and legal system collaboration to support justice-involved individuals. Requirements
- Graduation from an accredited nursing program.
- Must be a Registered Nurse (RN) currently licensed to practice in the state of North Carolina.
- CPR/First Aid certification required.
- Valid Driver’s License and Reliable Transportation required.
Nice To Haves
- Crisis Prevention Intervention (CPI) certification required within 90 days of hire.
- Experience in crisis intervention, emergency psychiatry, or correctional nursing.
- Knowledge of criminal justice system processes, forensic risk assessment, and reentry planning.
- Experience working with justice-involved individuals with mental illness and co-occurring disorders.
Responsibilities
- Administer and monitor medications, provide health education, and ensure adherence to prescribed treatments.
- Conduct health assessments and screenings to identify physical and psychiatric needs requiring medical or clinical intervention.
- Collaborate with criminal justice and behavioral health partners to coordinate medical and psychiatric care for justice-involved individuals.
- Support reentry and continuity of care for individuals transitioning from incarceration to community-based services.
- Participate in multidisciplinary team meetings, providing medical input, crisis support, and documentation in accordance with regulatory standards.
- Provide care and manage medications for people with mental health and substance use needs.
- Monitor clients’ health and respond to mental health or other urgent crises.
- Coordinate care with law enforcement, courts, probation/parole, and correctional services.
- Support clients returning to the community and connect them with ongoing services.
