About The Position

Carolina Outreach is seeking a Forensic Assertive Community Treatment (FACT) Registered Nurse to deliver medical and psychiatric care to individuals with serious mental illness and co-occurring substance use disorders who are involved in the criminal justice system. As a Forensic ACT RN, you will provide medication management, health monitoring, crisis intervention, and care coordination while working closely with law enforcement, courts, probation/parole, and correctional healthcare providers to ensure continuity of care and successful community reintegration. This role emphasizes forensic treatment planning, risk assessment, and legal system collaboration to support justice-involved individuals. This description outlines the general scope and level of work for this role. It is not intended to include every duty or responsibility that may be required of employees in this position.

Requirements

  • Graduation from an accredited nursing program.
  • Must be a Registered Nurse (RN) currently licensed to practice in the state of North Carolina.
  • CPR/First Aid certification required.
  • Valid Driver’s License and Reliable Transportation required.

Nice To Haves

  • Crisis Prevention Intervention (CPI) certification required within 90 days of hire.
  • Experience in crisis intervention, emergency psychiatry, or correctional nursing.
  • Knowledge of criminal justice system processes, forensic risk assessment, and reentry planning.
  • Experience working with justice-involved individuals with mental illness and co-occurring disorders.

Responsibilities

  • Administer and monitor medications, provide health education, and ensure adherence to prescribed treatments.
  • Conduct health assessments and screenings to identify physical and psychiatric needs requiring medical or clinical intervention.
  • Collaborate with criminal justice and behavioral health partners to coordinate medical and psychiatric care for justice-involved individuals.
  • Support reentry and continuity of care for individuals transitioning from incarceration to community-based services.
  • Participate in multidisciplinary team meetings, providing medical input, crisis support, and documentation in accordance with regulatory standards.
  • Provide care and manage medications for people with mental health and substance use needs.
  • Monitor clients’ health and respond to mental health or other urgent crises.
  • Coordinate care with law enforcement, courts, probation/parole, and correctional services.
  • Support clients returning to the community and connect them with ongoing services.
