About The Position
Grace Community Care & Homes is developing its Medical and Preventive Care department and seeks a Registered Nurse Outpatient Clinic. The role contributes to person-centered services, reliable operations, and accurate documentation. The position is planned, and its start date and duties are contingent on applicable program approvals, funding, credentialing, and staffing readiness. Services may begin only within the approved scope and at approved locations. The position is based in New Jersey, with administrative coordination through Grace and on-site or community assignments as applicable. Weekly hours and the start date will be discussed during hiring. Grace welcomes qualified applicants and provides reasonable accommodations.
Requirements
- Active New Jersey RN license.
- Outpatient nursing and patient-education skills.
- Current role-appropriate life-support training.
- Provide relevant credentials, experience, availability, and a resume.
- Final regulatory eligibility will be verified before appointment or service delivery.
Responsibilities
- Provide nursing assessment, patient education, care coordination, and clinical support.
- Maintain accurate documentation and escalate changes in condition.
- Collaborate with department leadership, intake, credentialing, billing, and quality teams.
- Maintain confidentiality, respect participant choice, and communicate service gaps promptly.
- Follow role-specific policies and professional scope.
- Participate in orientation, competency review, and quality improvement.
