Registered Nurse or Licensed Practical Nurse Case Manager
About The Position
Boston Mountain Rural Health Center (BMRHC) is seeking a dedicated and caring Case Manager to join their clinical team at the Yellville Clinic. This full-time position focuses on chronic disease management, patient education, preventive care, and empowering individuals to take control of their health and wellness. The Case Manager will work closely with providers to deliver high-quality primary care services, supporting patients on their healthcare journey. Requirements
- Current Arkansas RN or LPN license
- Basic Life Support (BLS) Certification
- Some travel required
Responsibilities
- Assisting providers in the delivery of primary healthcare services
- Providing health education and chronic disease management support
- Educating patients on diabetes care and preventive health services
- Coordinating care and serving as a case manager for patients with chronic conditions
- Performing designated nursing procedures within the Arkansas State Board of Nursing scope of practice
