About The Position

This role involves providing on-call nursing services primarily during weekends, from Friday 5 PM to Monday 9 AM. Responsibilities include conducting home visits for patient assessment, treatment, and support, which may involve pronouncement of patient death, crisis management, and admission planning. The nurse will develop and revise nursing care plans in conjunction with the patient's physician and the Interdisciplinary Care Team (IDG), utilize excellent customer care skills, and prepare clinical and progress notes. The position also requires informing physicians and other personnel of patient needs and outcomes, timely submission of all documentation, and ensuring compliance with all relevant laws and regulations.

Requirements

Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
Current State Registered Nurse License
Current Driver's License
Reliable transportation
Provide ongoing valid and current auto liability insurance
Two years of experience as an RN preferred in a healthcare/clinical setting

Responsibilities

Provide home visits as needed for patient assessment, treatment, and support.
Perform pronouncement of patient death, crisis management, and admission.
Develop and/or revise nursing care plans, including goals, in conjunction with the patient’s physician and IDG.
Utilize excellent customer care skills with internal and external customers, including using proper phone etiquette.
Prepare clinical and progress notes that demonstrate progress toward established goals.
Inform physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care.
Ensure timely submission of all documentation.
Perform other duties as required.
Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
