About The Position

We are currently recruiting for FULL and PART time Registered Nurses (RN) for our Maternal Child Health Home Visiting Program - 5 year grant funded. Position Summary: The Registered Nurse (RN) Home Visitor provides evidence-based maternal and child health services through home visits to families with newborns. The RN conducts comprehensive assessments of maternal, infant, family, and environmental needs; provides health education and support; identifies risks; and connects families with community resources and healthcare services. The RN promotes healthy pregnancies, positive birth outcomes, infant well-being, and family resilience through a strengths-based, family-centered approach.

Requirements

  • Current unrestricted Connecticut Registered Nurse license.
  • Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN).
  • Minimum of one year of nursing experience.
  • Valid driver's license and reliable transportation.
  • Ability to travel throughout the service area and conduct home visits.

Nice To Haves

  • Experience in maternal-child health, labor and delivery, postpartum, newborn nursery, pediatrics, public health, community health, or home health preferred.
  • BSN degree.
  • Experience working with diverse populations and underserved communities.
  • Bilingual (English/Spanish) proficiency.
  • Experience with maternal mental health screening and community-based nursing.
  • Knowledge of local healthcare and social service resources.

Responsibilities

  • Conduct home visits with families during the prenatal and postpartum periods according to program guidelines.
  • Complete comprehensive assessments of maternal health, newborn health, family functioning, and home safety.
  • Evaluate physical, emotional, developmental, and psychosocial needs of mothers, infants, and caregivers.
  • Develop individualized recommendations and referral plans based on identified needs.
  • Provide follow-up support and care coordination as indicated.
  • Assess maternal postpartum recovery, including physical and emotional health.
  • Screen for postpartum depression, anxiety, and other mental health concerns.
  • Assess infant growth, feeding, sleep patterns, development, and overall health status.
  • Identify signs and symptoms requiring referral to healthcare providers.
  • Monitor family adjustment to the newborn and parenting challenges.
  • Provide education regarding: Newborn care and safety, Breastfeeding and infant feeding, Safe sleep practices, Maternal self-care and recovery, Child development and developmental milestones, Immunizations and preventive care, Injury prevention and home safety.
  • Support parents in developing confidence and parenting skills.
  • Reinforce healthy family practices and positive parent-child interactions.
  • Identify social, behavioral health, medical, and environmental needs.
  • Facilitate referrals to community resources, healthcare providers, mental health services, early intervention programs, and social service agencies.
  • Collaborate with Community Health Workers and interdisciplinary team members to address family needs.
  • Follow up on referrals to ensure families successfully access services.
  • Complete accurate, timely, and thorough documentation of all home visits and assessments.
  • Maintain electronic health records and program databases according to agency policies.
  • Participate in case reviews, supervision, staff meetings, and quality improvement activities.
  • Adhere to program standards, nursing practice regulations, confidentiality requirements, and safety protocols.
  • Establish and maintain positive relationships with families and community partners.
  • Participate in ongoing training and professional development activities.
  • Promote cultural humility, equity, and trauma-informed care in all interactions.
  • Represent the program professionally within the community.
