About The Position

If you are a registered nurse (RN) who desires to take the next step in the career ladder and use your experience to mentor, train, and educate your own team of caregivers, while coordinating and overseeing the care of your patients, the role of registered nurse (RN) nurse case manager may be your next calling. At Dove Healthcare, you will work with employees, patients, family members, and other professionals to coordinate all aspects of providing person-centered care. Dove Healthcare consists of 11 skilled nursing and rehabilitation centers, six assisted living residences, and three independent living complexes in Barron, Bloomer, Chippewa Falls, Eau Claire, Fennimore, Lodi, Osseo, Rice Lake, Spooner, St. Croix Falls, and Superior. Our core services include inpatient and outpatient rehabilitation, post-hospital care, ventilator care, skilled nursing, long-term and end-of-life care, as well as senior independent living, assisted living, and memory care. Requirements

current licensure in the State of Wisconsin as a registered nurse (RN)
degree from an accredited school of nursing
previous experience in related field
ability to prioritize duties in a constantly changing environment
effective management skills
customer service oriented with patients, employees, families, and other agencies
good problem-solving skills
excellent time management skills
ability to accept and give directions
work well in a team environment
a desire to make a difference in people's lives

Responsibilities

supervise the day-to-day activities of assigned residents and employees
coordinate, plan, implement, and evaluate the nursing services provided to each resident using clinical competence
review and audit compliance with the patient's plan of care and changing needs
ensure appropriate staffing patterns are in place
evaluate employee duties and needs
hold employees accountable for following policies and procedures
provide a safe, comfortable, and home-like environment
on-call rotation
other duties as assigned
