This role involves providing quality patient care in the comfort of the patient’s home, in accordance with the physician-established plan of care, and collaborating with the patient, family, and care team to support the patient’s health, safety, comfort, and quality of life.
Requirements
- Current RN or LPN license applicable to the state of Arizona.
- Strong clinical, communication, and organizational skills.
- Compassionate and patient-centered approach to care.
- Ability to work independently and follow an established plan of care.
Nice To Haves
- Previous home health or in-home nursing experience preferred.
Responsibilities
- Provide nursing care in the patient’s residence in accordance with the physician-established plan of care.
- Monitor the patient’s condition and report significant changes as appropriate.
- Perform treatments and administer medications as ordered and within the nurse’s scope of practice.
- Provide patient and family education as appropriate.
- Maintain accurate and timely clinical documentation.
- Communicate effectively with the patient, family, physician, and other members of the care team.
- Promote patient safety, comfort, dignity, and independence.
- Follow applicable professional, regulatory, and agency requirements.
