About The Position

This role is for a Registered Nurse Lead Case Manager at Ascension St. John's Medical Center in Tulsa, OK, within the Case Management department. The position is full-time, with working hours from Monday to Friday, 8 am to 4:30 pm. The role involves mentoring junior case managers, improving case management processes by analyzing data and identifying trends, preventing readmissions through detailed discharge planning, and ensuring continuity of care for patients from admission to discharge.

Requirements

• Licensed Registered Nurse credentialed from the Oklahoma Board of Nursing obtained prior to hire date or job transfer date required.
• High school diploma/GED with 2 years of experience, OR Associate's degree, OR Technical degree required.
• 5+ years of RN experience
• 3+ years of Case Management experience

Nice To Haves

• Case Manager credentialed from the American Case Management Association (ACMA) preferred.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
• Master's degree preferred.
• Hospital discharge planning experience preferred.

Responsibilities

• Mentor junior case managers with complex cases and prioritizing their tasks
• Review case management data to identify trends and enhance workflows
• Develop detailed discharge plans to lower 30-day readmission rates
• Support patients from admission through discharge to prevent care gaps
