About The Position

ChristianaCare's Primary Care at Home is seeking an experienced Registered Nurse II to deliver high-quality, safe, and evidence-based patient care. This role is responsible for achieving optimal patient outcomes through both independent practice and collaborative teamwork within the clinical setting. The Registered Nurse II will apply the Nursing Process to assess, plan, implement, and evaluate care, contributing as a trusted and integral member of the healthcare team. Our dedicated team delivers high-quality, patient-centered care directly in the home. Patients are supported by experienced and compassionate professionals, including physicians, nurse practitioners, physician assistants, and nurses, who provide in-home medical visits. Through regularly scheduled appointments, our team builds meaningful relationships with patients, caregivers, and families—helping to enhance comfort, trust, and overall quality of life for homebound individuals. This role is split between triaging for the practice as well as home visits.

Requirements

  • A graduate of a state approved or National League of Nursing accredited School of Nursing.
  • Registered Nurse licensure or eligible for licensure in the state Delaware, Maryland or Pennsylvania & New Jersey depending on location of employment.
  • Two years’ experience as a Registered Nurse, physician office experience is preferred.
  • Basic Life Support (BLS) required.
  • Proof of a valid driver’s license and active auto insurance is required.

Nice To Haves

  • Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (AND) will be considered with an agreement of completion of BSN in 3 years

Responsibilities

  • Provide age-appropriate, patient-centered care to pediatric through geriatric populations in both virtual and in-person settings, using strong clinical knowledge to assess and address patient needs.
  • Deliver services such as wellness visits, care management, point-of-care testing, and triaging patients.
  • Collaborate closely with providers to support examinations, procedures, and efficient clinic operations.
  • Serve as a key liaison among patients, providers, and staff, ensuring clear communication, coordinating workflow, and responding to urgent situations.
  • Support staff development through training and mentorship.
  • Participate in quality and compliance initiatives.
  • Contribute to process improvement efforts—all while maintaining a strong focus on safety, patient education, and high-quality care delivery.
