About The Position

A Clinical Nurse II (CN II) is a competent clinician who provides nursing care in a skilled and effective manner. A CNII Independently prioritizes & delivers dependable and effective patient care in a variety of patient situations using nursing process utilizing appropriate decision-making skills. A CNII exhibits competence in the technical, interpersonal and cognitive skills required to independently and safely manage routine patient assignment. The Clinical Nurse II begins to develop their mentorship skillsets under the leadership of experienced and expert clinicians on their unit. Drives the mentee-mentor relationship to identify gaps in skills, practice and knowledge to achieve desired goals. A CNII establishes a plan of care, sets goals, determines priorities, and provides care within established standards of practice. The scope of responsibility, duties and accountability at the facility department and position levels including knowledge and practice of the clinical/technical skills necessary to the position consistent with the Nurse Practice Act. Nursing job duties are to meet the Nurse Practice Act, Title 22, The Joint Commission and the Plan for Provision of Nursing Care and the Plan for Provision of Care to Patients, and other regulatory requirements. Collaborates to problem solve and make decisions to achieve desired outcomes Establishes effective working relationships with cross-functional team(s) Responds timely, effectively and appropriately to deliverables Shares knowledge, time and expertise to assist other members of the team Initiate, plan, design, implement, and monitor project(s) Ability to manage projects, establish timeliness, analyze data relevant to areas of responsibility. This is a Diabetes Quality Improvement nursing position. The Diabetes QI nurse will be the departmental resource, promoting enterprise-wide diabetes quality improvement. Focus is on new areas of diabetes technology, devices and process improvement in multiple areas. Although position is based in DOTEC clinic, projects are in the ambulatory, inpatient and other specialty spaces and can encompass transitions of care. Interest in Technology and Process Improvement are necessary for success in this position. Leads pre-visit planning and clinic preparation with diabetes device data to optimize clinic operations; reviews schedules, communicates with patients, ensures device data readiness, and supports effective use of clinical data for patient visits. Medicine – Supports current EMR integration of diabetes devices (e.g., Glooko, Libre), including workflow setup for clinic staff, patients, and providers and leads the new integrations that come down the pipeline. Under the Quality Improvement team, partners with vendors, EIS, and clinical stakeholders to implement diabetes technology (e.g., CGM); develops workflows, analyzes data, and drives program improvements and sustainability. Educates patients and clinic staff in diabetes technology specifically sensors. Develops and standardizes educational materials for clinic and inpatient settings; coordinates programs for patients, caregivers, and staff, including support for inpatient diabetes initiatives (Diabetes STAR program).
