This role delivers quality care to patients through the nursing process within the home setting and virtually, supporting acute care delivery through in-home visits and virtual monitoring. The nurse serves as an advocate, providing emotional and informational support, and involving patients and families in their care plan. They also plan for comfort measures, collaborate with healthcare professionals to optimize outcomes, and educate patients and families about medical conditions.
