About The Position

This is a travel assignment for a Registered Nurse specializing in hospice care. The role involves providing in-patient hospice care, managing patient symptoms, and collaborating with an interdisciplinary team. The assignment is for 12 weeks in Grand Junction, Colorado.

Requirements

Active Registered Nurse license (state-specific as required for assignment)
Ability to practice in a hospice care setting and participate in on-call rotations
Strong assessment, critical thinking, and interpersonal communication skills
Demonstrated ability to work with an interdisciplinary team and manage a patient caseload
Commitment to patient- and family-centered hospice care

Nice To Haves

Prior hospice or palliative care experience is preferred but not mandatory

Responsibilities

Assess patients and manage hospice symptoms, implementing individualized care plans with the interdisciplinary team
Deliver comprehensive nursing care to an assigned caseload, including medication administration, vital signs, and symptom control
Educate patients and families on goals of care, treatment options, and self-care strategies to support informed decision-making
Collaborate with physicians, social workers, chaplains, and other clinicians; coordinate care across home, nursing home, group home, or assisted living settings
Document care plans, progress notes, and care conferences; ensure accurate and timely communication within the team
Maintain compliance with hospice regulations and organization policies; participate in quality and safety initiatives
