About The Position

Amedisys is hiring a full-time Registered Nurse Home Health case manager serving Central Anne Arundel County, including Glen Burnie, Pasadena, Millersville, and Linthicum. The nurse helps patients heal at home by assessing their needs, collaborating with the care team on plans of care, and supporting patient health and independence through teaching and appropriate rehabilitative measures. The role also includes supervising LPNs and HHAs, maintaining timely and accurate documentation, communicating patient progress, providing staff education, participating in continuing education, and completing assigned on-call responsibilities. The advertised pay range is $87,000–$108,000. Compensation may vary by geographic market, job-related knowledge, skills, and experience. Requirements

At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP Clinical.
Current RN license specific to the state(s) assigned.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.
Candidates with less than six months of clinical RN experience must participate in the RN Intern program.

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Make referrals to other disciplines as indicated by patient needs, or document the rationale for not making referrals.
Promote patient health and independence through teaching and appropriate rehabilitative measures, including helping patients learn self-care techniques.
Supervise LPNs and HHAs.
Complete documentation accurately and on time.
Regularly communicate patient progress to the clinical manager and care team.
Plan and provide staff education.
Perform assigned on-call responsibilities and provide on-call services to patients and families.
Participate in clinical development and continuing education programs.
Perform other duties as assigned.
