About The Position

Elara Caring is seeking a Registered Nurse Home Health to provide compassionate nursing care to patients in their homes. The nurse assesses patients, delivers care according to plans of care and physician orders, monitors patient conditions, and works with the care team to support quality, safe, and continuous care. The role also includes protecting patient and staff privacy in accordance with HIPAA and educating patients and families about care and safety.

Requirements

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates, or Baccalaureate School of Nursing.
• At least 1 year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license and maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred, but not required.

Responsibilities

• Assess patients to determine their care needs.
