About The Position
As part of Henry Ford Health's Community Care Services, our Home Health Care team plays a vital role in expanding access to care by delivering essential health services directly to patients in their homes. We focus on meeting individuals where they are, providing care coordination, and connecting them with resources such as physical therapy, occupational therapy, and social work. Our mission is to make a meaningful impact across diverse populations, ensuring integrated and compassionate care for those we serve. As an RN in Home Health Care, you will independently provide direct patient care in the home, utilizing your nursing expertise to assess and address patient needs. You will coordinate with allied health professionals to ensure a comprehensive care plan, supporting patients and their families in achieving optimal health outcomes. Your role is critical in delivering personalized, high-quality care that improves quality of life.
Requirements
- Graduation from an accredited Registered Nurse program.
- Current licensure as a Registered Nurse in the State of Michigan.
- Ability to effectively communicate with patients, families, and healthcare personnel.
- Knowledge of computer use for electronic documentation.
- Strong interpersonal and customer service skills.
- Valid driver's license, satisfactory driving record, and reliable transportation.
- Ability to lift, carry, transport, and move patients as needed.
Nice To Haves
- Minimum of nine months to one year of clinical nursing experience.
Responsibilities
- Provide direct patient care in the home, applying nursing principles to assess and meet patient needs.
- Coordinate with allied health services such as physical therapy, occupational therapy, and social work to ensure integrated care.
- Document patient care electronically, maintaining accurate and up-to-date records.
- Communicate effectively with patients, families, and healthcare personnel to ensure seamless care delivery.
- Transport and assist patients as needed, ensuring their safety and comfort.
