About The Position
Join our Community Care Services team at Henry Ford Health, where we are dedicated to expanding access to care through community-based programs. Our mission is to meet patients where they are, providing essential health resources, support services, care coordination, home health care, and hospice services. We strive to make a meaningful impact across diverse populations, ensuring that every individual receives the care they need in the comfort of their own home. As an RN in Home Health Care, you will independently provide direct patient care in the home, utilizing your nursing expertise to assess and meet patient needs. You will coordinate with allied health care services to ensure an integrated care delivery system, supporting patients and their families in achieving optimal health outcomes.
Requirements
- Current licensure by the State of Michigan as a registered nurse
- Minimum of nine (9) months of clinical nursing experience required.
- Must possess a current State of Michigan driver's license and a satisfactory driving record.
- Must have reliable transportation and a telephone.
Responsibilities
- Provide direct patient care in the home, applying nursing principles to assess and address patient needs.
- Coordinate with allied health care services such as home health aides, physical therapy, occupational therapy, dietetics, speech-language pathology, and social work.
- Ensure an integrated home health care delivery system by collaborating with healthcare professionals and families.
- Document patient care electronically using computer systems.
- Communicate effectively with patients, families, and healthcare personnel to provide comprehensive care.
