As a Home Care Registered Nurse, you will provide skilled nursing services to patients in their homes, ensuring high-quality care that promotes healing, independence, and overall well-being. You will play a vital role in assessing patient needs, developing care plans, educating patients and caregivers, and coordinating services across the healthcare continuum. About the Department As part of Henry Ford Health's Community Care Services, our Home Health Care team plays a vital role in expanding access to care by delivering essential health services directly to patients in their homes. We focus on meeting individuals where they are, providing care coordination, home health services, and support across diverse populations. Our team is dedicated to making a meaningful impact by connecting patients with the resources they need to thrive in their communities. Role Overview As an RN in Home Health Care, you will independently provide direct patient care in the home, utilizing your nursing expertise to assess and address patient needs. You will coordinate with allied health professionals to ensure a comprehensive and integrated care delivery system, supporting patients and their families in achieving optimal health outcomes.

Requirements

Graduation from an accredited Registered Nurse program.
Current licensure as a Registered Nurse in the State of Michigan.
Ability to effectively communicate with patients, families, and healthcare personnel.
Knowledge of computer use for electronic documentation.
Current driver's license, satisfactory driving record, and reliable transportation.
Ability to lift, carry, transport, and move clients as needed.

Nice To Haves

Minimum of nine months to one year of clinical nursing experience.

Responsibilities

Provide direct patient care in the home, applying nursing principles to assess and meet patient needs.
Coordinate with allied health professionals to ensure integrated care delivery.
Document patient care electronically using computer systems.
