About The Position

Humber River Health is seeking a compassionate Registered Nurse (RN) for their Schulich Family Medicine Teaching Unit. As North America's first fully digital hospital, Humber River Health is dedicated to changing healthcare through technology and clinical expertise. This role focuses on delivering high-quality, safe care within a community-focused environment that values equity, inclusivity, and academic collaboration. The RN will work with Family Medicine residents and learners, integrating best practices and evidence-based research into patient care. The position is full-time, located at the Wilson Site, with 8-hour shifts (days, evenings, subject to change).

Requirements

  • Current Ontario College of Nurses Registration in good standing
  • Current BCLS certification required
  • Demonstrated experience supporting patient care including assessment, intervention, referrals and transitions as well as patient education/teaching
  • Demonstrated ability to work as a member of a multidisciplinary team, including learners
  • Ability to adapt and effectively work in a fast-paced environment with changing priorities and time sensitive targets
  • Must possess excellent organizational skills
  • Exhibit advance assessment skills and ability to identify and react quickly to a deteriorating patient.
  • Demonstrates excellent patient and family teaching skills that encompass adult learning principles.
  • Demonstrates effective problem solving skills and an ability to work well independently with minimal supervision.
  • Conducts oneself with professionalism and respect at all times.
  • Highly effective interpersonal and communication skills are required.
  • Sensitivity to and awareness of cultural, racial, economic and socially diverse communities.
  • Excellent attendance and discipline free record required.

Responsibilities

  • Assesses, plans, implements and evaluates patient care in alignment with the RN scope of practice outlined by the College of Nurses of Ontario.
  • Maintains the health record according to CNO documentation standards and Humber River Health Policies and Procedures.
  • Works collaboratively with Family Medicine residents and learners, including teaching and assisting in education.
  • Integrates best practice guidelines and evidence based research into clinical practice related to family medicine and care along the continuum.
  • Conducts patient intake and review of intake forms for accuracy and completeness.
  • Receives patient calls via nursing line, assess patient phone calls, provides support, and responds to patient concerns in accordance with the standards established by the College of Nurses of Ontario.
  • Keeps abreast of new developments in family medicine and related evidence-based advancements with the intention of integrating into clinical practice.
  • Ensure patients are connected with the appropriate services, resources and supports within HRH and in the community.
  • Provides health education for patients and families regarding chronic disease management, diagnostic tests, prescribed treatments and management of their condition.
  • Works collaboratively with the patient’s primary care physician or primary care provider and community agencies as required throughout the care process.
  • Identifies and prioritizes not only the physical needs, but also the psychological, social and spiritual needs of the patient.
  • Ensures that the patient or caregiver is included in development of the care plan and understands the medical advice or recommendations.
  • Demonstrates the ability to navigate and perform computer competencies including use of the primary care EMR.
  • Works collaboratively with the interdisciplinary team to support the ongoing functioning of the outpatient Family Medicine Clinic related to referrals, consultations and follow-ups.
  • Participates in program planning.
