Requirements

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date reqired. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date required.
• Required professional licensure/certification AND 1 year of cumulative job specific experience required.

Responsibilities

• Lead individualized care journeys by executing complex medical plans with precision, ensuring every clinical decision prioritizes patient safety, comfort, and personal dignity.
• Serve as the central communication hub for the interdisciplinary team to facilitate seamless collaboration and ensure the delivery of holistic, well-integrated care.
• Drive optimal patient outcomes by proactively tracking health trends and documenting progress, enabling the medical team to provide timely, life-changing interventions.
• Empower patients and their families for life after discharge by leading comprehensive transition planning and providing the tools necessary for a successful recovery at home.
