JOB SUMMARY To provide quality and effective diabetes education and care coordination services in a community clinic within the scope of training, certification, and licensure. This position is responsible for overseeing the day-to-day operations of the clinic's DSMES service, ensuring that the National Standards (NSDSMES) are met and ADA Recognition is maintained.

Requirements

  • Registered Nurse licensed in the State of Minnesota required
  • CDCES Certification required.
  • Experience working in American Indian communities preferred
  • 2+ years of experience providing care coordination services and health education preferred
  • Experience leading Quality Improvement activities
  • Excellent verbal and written communication skills, including EMR documentation
  • Actively participates and works positively, flexibly and cooperatively in a team effort to accomplish the goals of the Indian Health Board of Minneapolis.

Responsibilities

  • Act as Health Care Home Care Coordinator, seeking to understand and meet the needs of the patient through respectful and caring interactions with patients, their families and other health professionals.
  • Serve as a linkage between patients and their internal and external healthcare and service providers to improve coordination of services and information flow to ensure effective treatment plans.
  • Identify patient needs and create relationships with community partners to facilitate opportunities for patient's referrals and resources (i.e. podiatrist, endocrinologist, ophthalmologist, etc.).
  • Implement daily clinic activities to ensure outstanding patient care and care coordination is provided.
  • Facilitate diabetes clinic including student needs and workflow.
  • Provide excellent customer service to patients, peers, leadership, and community partners.
  • Actively involved in Special Program for Diabetes grant reports such as annual progress reports and renewal applications.
  • Actively participate in all areas of maintaining American Diabetes Association Recognition including renewal and certification.
  • Plans, coordinates and provides patient-specific health education based on the chronic or associated conditions identified such as obesity, smoking, high blood pressure, diabetes, and improving preventative screening such as cancer screening.
  • Works with the patient, providers, and Health Care Home Care Team members to develop and maintain an individualized clinical Care Plan for the patient, reviewing regularly and updating as needed.
  • Assists patients with the identification, selection, monitoring and documentation of self-management goals, monitoring the patient's progress toward goal achievement and modifying as needed.
  • Complete psychosocial assessments with patients and serve as an advocate for the patient to understand needs (i.e. shelter, transportation, child care, safety) and arrange or coordinate applicable services.
  • Complete and follow up on Prior Authorizations related to diabetes care.
  • Knowledgeable about insulin delivery systems including continuous glucose monitoring
  • Provides ongoing continuous glucose monitoring training to applicable medical staff.
  • Partners with area colleges and agencies to provide students with a community wellness experience.
  • Provide group education using approved curriculum for patients with diabetes and families in accordance with Indian Health Service and American Diabetes Association guidelines and recommendations.
  • Review and organize educational materials for the diabetes program on a regular basis to ensure up-to-date, culturally relevant materials are provided for patients.
  • Provide individual diabetes education at the request of the medical providers including home glucose monitor instruction, continuous glucose monitoring, medicine, insulin delivery systems, carbohydrate counting, and lifestyle instruction.
  • Assist Supervisor with implementing performance improvement audits and initiatives including Indian Health Service annual audits, American Diabetes Association internal audits, Minnesota Community Measurement, Uniform Data Systems and Health Care Home.
  • Organize and participate in diabetes educational events.
  • Lead efforts to ensure that the Electronic Health Records (EHR) System meets the needs of clinical staff to document diabetes visits and diabetes education.
  • Maintain patient information in the EHR.
  • Represent the Indian Health Board at meetings and with other organizations as appropriate.
  • Other duties as assigned by supervisor.
