This Registered Nurse Certified Diabetes Care and Education Specialist (CDCES) position is responsible for providing quality and effective diabetes education and care coordination services within the clinic. The role involves overseeing the day-to-day operations of the clinic's Diabetes Self-Management Education and Support (DSMES) service, ensuring compliance with National Standards (NSDSMES) and maintaining ADA Recognition.

Requirements

  • Registered Nurse licensed in the State of Minnesota required.
  • CDCES Certification required. Candidates on-track to attain CDCES will be considered.
  • Experience working in American Indian communities preferred.
  • 2+ years of experience providing care coordination services and health education preferred.
  • Experience leading Quality Improvement activities.
  • Excellent verbal and written communication skills, including EMR documentation.
  • Actively participates and works positively, flexibly, and cooperatively in a team effort.
  • Manages time effectively and prioritizes task completion to meet deadlines.

Responsibilities

  • Act as Health Care Home Care Coordinator, understanding and meeting patient needs through respectful interactions.
  • Serve as a liaison between patients and their healthcare/service providers to improve coordination and ensure effective treatment plans.
  • Identify patient needs and build relationships with community partners for referrals and resources (e.g., podiatrist, endocrinologist, ophthalmologist).
  • Implement daily clinic activities to ensure outstanding patient care and care coordination.
  • Facilitate the diabetes clinic, including student needs and workflow.
  • Provide excellent customer service to patients, peers, leadership, and community partners.
  • Actively participate in Special Program for Diabetes grant reports (annual progress reports, renewal applications).
  • Actively participate in all aspects of maintaining American Diabetes Association Recognition, including renewal and certification.
  • Plan, coordinate, and provide patient-specific health education for chronic conditions (obesity, smoking, high blood pressure, diabetes) and preventative screenings.
  • Collaborate with patients, providers, and Health Care Home Care Team members to develop and maintain individualized clinical Care Plans.
  • Assist patients with identifying, selecting, monitoring, and documenting self-management goals, and modifying them as needed.
  • Complete psychosocial assessments and advocate for patients' needs (shelter, transportation, childcare, safety), arranging or coordinating services.
  • Complete and follow up on Prior Authorizations related to diabetes care.
  • Provide ongoing continuous glucose monitoring training to applicable medical staff.
  • Partner with area colleges and agencies to provide community wellness experiences for students.
  • Provide group education using approved curriculum for patients with diabetes and their families, adhering to Indian Health Service and American Diabetes Association guidelines.
  • Review and organize educational materials for the diabetes program to ensure they are up-to-date and culturally relevant.
  • Provide individual diabetes education as requested by medical providers, including instruction on home glucose monitoring, continuous glucose monitoring, medications, insulin delivery systems, carbohydrate counting, and lifestyle.
  • Assist Supervisor with implementing performance improvement audits and initiatives (e.g., IHS annual audits, ADA internal audits, Minnesota Community Measurement, UDS, Health Care Home).
  • Organize and participate in diabetes educational events.
  • Lead efforts to ensure the Electronic Health Records (EHR) System meets clinical staff needs for documenting diabetes visits and education.
  • Maintain patient information in the EHR.
  • Represent the Indian Health Board at meetings and with other organizations.
  • Perform other duties as assigned by the supervisor.
