About The Position
Roze Room is a leading provider of Hospice and Palliative Care, celebrating 25 years of service to Southern California communities. This Full-Time Hospice RN Case Manager (RNCM) role is field-based and serves the San Gabriel Valley areas. The position offers a competitive hourly rate of $50/hour plus mileage reimbursement. This role is about guiding patients and families through life's vulnerable transitions, delivering comfort, dignity, and compassionate care at home.
Requirements
- Licensed Registered Nurse (RN)

- Experienced in hospice, home health, or case management
Responsibilities
- Lead and coordinate individualized hospice care plans
- Provide skilled nursing care including pain and symptom management
- Supervise LVNs and CHHAs in the field
- Collaborate with physicians and the interdisciplinary team
- Educate and support patients and families during critical moments
