About The Position
Health Services of Northern New York is seeking a Registered Nurse Case Manager to manage client care in the home setting. The role includes performing nursing assessments, coordinating care with physicians, and overseeing the involvement of additional disciplines for each client, which may include Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Worker, and Home Health Aides. The position requires travel to and from patient homes throughout the assigned region. The ideal candidate has excellent communication skills and is comfortable traveling to and working one-on-one with patients in their homes.
