About The Position

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide individualized, holistic support to patients in their homes. The role serves as a cornerstone of patient care, collaborating with the care team to develop personalized plans, lead end-of-life conversations, and provide compassionate, clinically excellent symptom management. The position offers meaningful patient connections and autonomy in practice.

This is a full-time role based in Lincoln, Nebraska, with a schedule of 8:00 a.m. to 5:00 p.m. and on-call expectations. Travel is listed as 100%.

Requirements

• Associate, bachelor's, or master's degree from an accredited nursing program and Registered Nurse (RN) qualification.
• At least 1 year of recent acute care experience.
• Reliable transportation, a valid driver's license, and proof of auto insurance.

• Hospice or home health experience.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patients and families, updating them as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
• Collaborate with the care team to support patient care.
• Lead end-of-life conversations and provide symptom management.
