About The Position

Ascension St. Vincent's Riverside Hospital is hiring a Registered Nurse Case Manager for the Evening Shift on a Part-Time basis. This role involves orchestrating the full interdisciplinary plan of care from admission through discharge to ensure a seamless patient experience. The Case Manager will design personalized care strategies that integrate clinical requirements with patient and family values and preferences. Responsibilities include monitoring patient progress, overseeing medical documentation to ensure treatment options and services align with diagnostic needs, advocating for essential resources, proactively removing barriers to ensure the effective delivery of the health plan, and maintaining continuous collaboration with supervisors and providers to optimize care implementation and post-discharge success.

Requirements

Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.

Nice To Haves

BLS Provider preferred. American Heart Association or American Red Cross accepted.
