About The Position

As the Per Diem Registered Nurse Case Manager with Heart N Home Hospice, a part of LHC Group and the Optum family of businesses, you will assume nursing responsibility for delivering the Plan of Care for each hospice patient. You will evaluate your performance and make necessary changes to increase productivity and the quality of care, while adhering to and supporting hospice policies, procedures, goals, and objectives. The role focuses on personalized care and supporting patients and families through each stage of their health journey in the home setting.

Requirements

• Current unrestricted RN licensure in the state of practice.
• At least 1 year of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.

Nice To Haves

• Current CPR certification or ability to complete it within 90 days of hire.

Responsibilities

• Conduct an initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify patients' and families' physical, psychosocial, and environmental needs, and reassess as needed at least every 15 days.
• Initiate and coordinate the plan of care while maintaining accurate, relevant clinical notes about the patient's condition.
• Document problems, appropriate goals, interventions, and the patient and family's response to hospice care.
• Collaborate daily with the patient and family, attending physician, and other members of the interdisciplinary group (IDG) in providing care.
• Evaluate personal and professional performance and make necessary changes to improve productivity and quality of care.
