This specific role is for a Registered Nurse, Case Manager at the Baptist Medical Jacksonville - Downtown campus. It is a Full Time Day shift position, Monday through Friday, within the In-Patient Case Management Department. The Case Manager is responsible for care coordination for hospitalized patients, conducting high-risk screenings and assessments, coordinating post-acute care services with documentation, and collaborating with multidisciplinary team members.
Requirements
- 1 year case management experience
- Associate's Degree Nursing
- Basic Life Support (BLS)
- Driver's Licensed (DMV)
- Active licensed Registered Nurse (RN)
Responsibilities
- Care coordination for hospitalized patients.
