About The Position

As a Registered Nurse Case Manager, you will provide direct care to patients in their homes or care facilities. The role includes listening attentively to patients’ needs, respecting their wishes, and building trusting relationships with patients and their families. You will serve as the primary communication link within an interdisciplinary team of holistic care providers. Requirements

• At least six months of prior skilled nursing experience.
• An active, unencumbered state Registered Nurse license.
• A current driver’s license and auto liability coverage.
• Interest and ability in providing patient care in homes and facilities.
