About The Position

At St. Croix Hospice, the Registered Nurse Case Manager provides one-to-one holistic support to patients and families in their homes. The role partners with an interdisciplinary team to develop individualized care plans, guide end-of-life conversations, and help ensure patients’ comfort with professionalism and empathy. St. Croix Hospice is committed to helping patients make the most of their time with dignity, comfort, and respect. This is a full-time position based in Moline, Illinois, with an 8:00 a.m. to 5:00 p.m. schedule and on-call expectations. The position has a 100% travel percentage and requires reliable transportation, a valid driver’s license, and proof of auto insurance. The stated annual base salary range is $85,000–$90,000. Actual salary may depend on factors including location, education, certifications, experience, and skills. The posting also advertises a $5,000 signing bonus.

Requirements

Associate, bachelor’s, or master’s degree from an accredited nursing program and RN qualification.
At least 1 year of recent acute care experience.
Reliable transportation, a valid driver’s license, and proof of auto insurance.
Compassionate approach to patient care and ability to support patients and families.

Responsibilities

Complete initial and ongoing comprehensive assessments to determine hospice needs.
Develop individualized care plans with patients and families and update them as needs evolve.
Administer medications and treatments as prescribed.
Educate patients and caregivers on care techniques, medications, diet, and symptom management.
Rotate on-call duties and ensure seamless care transitions.
Partner with an interdisciplinary team to support patients and families and guide end-of-life conversations.
