Registered Nurse Case Manager Houston - Central /South East Houston
About The Position
Constellation Health Services is seeking a compassionate and skilled Registered Nurse Case Manager to care for hospice patients and their families in homes, assisted living facilities, and skilled nursing facilities in Central/South East Houston. The position provides skilled nursing care and coordinates patient care with the physician, family, caregivers, and other disciplines. It involves documenting care using a computer-based system, attending weekly interdisciplinary team meetings, and providing weekend and holiday coverage as needed.
Requirements
- Graduate of an accredited RN program.
- Hold a Registered Nurse license in the state of employment.
- At least 1–2 years of RN experience.
- Basic knowledge of the iOS operating system.
Nice To Haves
- Previous hospice care experience is strongly desired.
Responsibilities
- Provide skilled nursing care, including administering medications and treatments prescribed in the physician’s plan of care for patients in their home setting.
- Perform comprehensive assessments and coordinate the plan of care with all disciplines involved in the patient’s needs, including family and primary caregivers.
- Observe, evaluate, and report changes in patients’ physical and emotional status to the nursing supervisor and attending physician.
- Participate in weekly Interdisciplinary Team Meetings and provide updates on patients’ physical, emotional, psychosocial, and spiritual status.
- Maintain and process documentation in compliance with agency standards.
- Use a computer-based documentation system and submit required clinical documentation, time records, and other information within the required time frame.
- Provide input to the Home Health Aide Supervisor as needed for hospice aide evaluations.
- Provide weekend and holiday coverage as needed.
