Registered Nurse Case Manager (RN)
About The Position
Hospice of the West is seeking a full-time Hospice Registered Nurse Case Manager to provide end-of-life care throughout the community. The role delivers hands-on hospice nursing care, coordinates services, and guides patients and families through their hospice journey as part of an interdisciplinary care team. The position serves the Surprise, Sun City West, and Sun City Grand territories and follows a Monday–Friday, 8 a.m.–5 p.m. schedule, with shared call duties. Requirements
- Current, unencumbered Registered Nurse license.
- At least one year of RN experience in a medical-surgical or acute-care setting.
- Computer proficiency, including typing and use of various programs, with a background in electronic medical records (EMR).
- Current CPR certification if required by the state.
- Current driver’s license and vehicle insurance, and access to dependable transportation.

Responsibilities
- Respond to clinical referral information and address inquiries and requests.
- Partner with the Clinical Director to assess client eligibility and suitability for hospice services.
- Provide and coordinate direct nursing care focused on patient comfort and quality of life.
- Deliver nursing care using established principles and techniques, including care to ease pain.
- Educate and advocate for patients and families, supporting their participation in personalized care planning.
- Share responsibility for call duties.
