Registered Nurse Case Manager (RN)
About The Position
St. Croix Hospice is seeking a Registered Nurse Case Manager to provide compassionate, individualized, holistic support to patients in their homes. The role serves as a cornerstone of patient care, collaborating with the care team to develop personalized plans of care, lead end-of-life conversations, and provide exceptional symptom management with empathy and clinical excellence. The position offers meaningful patient connections and autonomy in practice. The position is full-time, based in Independence, Missouri, with a stated schedule of 8:00 a.m. to 5:00 p.m. and on-call expectations. Travel percentage is listed as 100%.
Requirements
- Associate, bachelor's, or master's degree from an accredited nursing program (RN).
- At least 1 year of recent acute care experience.
- Reliable transportation, a valid driver's license, and proof of auto insurance.
Nice To Haves
- Hospice or home health experience preferred.
Responsibilities
- Complete initial and ongoing comprehensive assessments to determine hospice needs.
- Develop individualized care plans with patients and families, updating them as needs evolve.
- Administer medications and treatments as prescribed.
- Educate patients and caregivers on care techniques, medications, diet, and symptom management.
- Rotate on-call duties and ensure seamless care transitions.
- Collaborate with the care team to provide individualized, holistic patient support.
- Lead end-of-life conversations and support exceptional symptom management.
