Registered Nurse (RN) Case Manager
About The Position
St. Croix Hospice is seeking a Registered Nurse Case Manager to provide one-to-one, whole-person care to patients and families in their homes. Working with an integrated care team, the nurse coordinates individualized care plans, supports families through complex decisions, and provides comfort with professionalism, empathy, and respect. The role offers meaningful patient connections and autonomy in nursing practice. The position is full-time, based in Roseville, Minnesota, and involves 100% travel. The scheduled hours are 8:00 a.m. to 5:00 p.m., with on-call expectations and rotating on-call duties. The annual base salary range is $80,000–$96,000; actual salary depends on factors including geographic location, education, certifications, experience, and skills.
Requirements
- Associate, bachelor's, or master's degree from an accredited nursing program and RN qualification.
- At least 1 year of recent acute care experience.
- Reliable transportation, a valid driver's license, and proof of auto insurance.
Nice To Haves
- Hospice or home health experience preferred.
Responsibilities
- Complete initial and ongoing comprehensive assessments to determine hospice needs.
- Develop individualized care plans with patients and families and update them as needs evolve.
- Administer prescribed medications and treatments.
- Educate patients and caregivers on care techniques, medications, diet, and symptom management.
- Rotate on-call duties and ensure seamless care transitions.
- Coordinate care with an integrated care team and support families through complex decisions.
