Registered Nurse (RN) – Home Health Case Manager $5,000 Sign-On Bonus | Full-Time & PRN | $65,000–$90,000/year Location: Jefferson City, MO and surrounding areas Are you a Registered Nurse (RN) looking for a rewarding home health nursing opportunity with better work-life balance? We’re looking for compassionate, skilled RNs / Registered Nurses to join our team as Home Health Case Managers serving Jefferson City and surrounding Missouri communities.
Requirements
- Missouri RN license or Compact RN license
- Valid driver’s license
- Reliable vehicle with current auto insurance
- Strong assessment, communication and clinical decision-making skills
- Ability to work independently while collaborating with an interdisciplinary care team
Nice To Haves
- Home health nursing experience preferred
- Critical care experience preferred
Responsibilities
- Manage an assigned home health nursing caseload
- Provide skilled nursing visits, evaluations and starts of care
- Supervise LPN visits and coordinate care among healthcare providers
- Complete comprehensive nursing assessments and develop individualized plans of care
- Communicate with physicians to obtain orders and establish diagnosis-driven plans of care
- Coordinate with insurance companies and other healthcare professionals
- Educate patients and families about conditions including CHF, COPD, diabetes, wound care and medication management
- Collaborate with Physical Therapy, Occupational Therapy and Speech Therapy
- Provide hands-on patient care in patients' homes
- Use clinical judgment, physical assessment skills and diagnostic information to support quality patient outcomes
