RN (Registered Nurse) Home Health Case Manager
About The Position
Home Health RN Case Manager — $78,000–$88,000 Wichita, KS | Full-Time, Monday–Friday | Home Health. This position offers a pay-per-visit model where the more visits completed, the more earned. Many nurses out-earn the top of the stated range. The role features a Monday–Friday schedule with on-call every 11-14 weeks, two weekday nights per month, and one holiday rotation per year. It provides autonomy in building the daily visit schedule, allowing for personal appointments. An AI tool, Roger, assists in reducing charting time during visits. Paid training for 4-6 weeks is guaranteed with pay. A ride-along opportunity is available before committing to employment. PTO accrues every pay period, offering 10 days plus 6 floating holidays from day one. Mileage is reimbursed for every visit, with additional pay for qualifying out-of-town visits.
Requirements
- Kansas or Compact RN license
- Valid driver's license
- Strong organizational skills
- Strong clinical skills
- Strong communication skills
Responsibilities
- Manage a caseload of home health patients.
- Supervise LPN visits.
- Coordinate with Physical Therapy, Occupational Therapy, and Speech Therapy teams.
- Conduct assessments.
- Build personalized care plans aligned with physician orders.
- Support patients managing conditions like CHF, COPD, diabetes, wounds, and medication regimens.
