RN (Registered Nurse) Home Health Case Manager
About The Position
Phoenix Home Care and Hospice is seeking a dedicated Home Health RN Case Manager in the Wichita, KS area. This full-time position offers a Monday-Friday schedule with a pay-per-visit model, allowing nurses to potentially earn above the base salary range. The role emphasizes real autonomy, with nurses building their own visit schedules and having charting support from an AI tool called Roger. New hires receive 4-6 weeks of paid training. The company offers a ride-along program for potential hires to experience a day with a current nurse before committing. Benefits include PTO that accrues from day one, mileage reimbursement, and out-of-town visit pay.
Requirements
- Kansas or Compact RN license
- Valid driver's license
- Strong organizational skills

Responsibilities
- Manage a caseload of home health patients.
- Supervise LPN visits.
- Coordinate with Physical Therapy, Occupational Therapy, and Speech Therapy teams.
- Conduct assessments.
- Build personalized care plans aligned with physician orders.
- Support patients managing conditions like CHF, COPD, diabetes, wounds, and medication regimens.
Benefits
- Base salary of $78,000–$88,000, with potential to earn more through a pay-per-visit model.
- Monday–Friday schedule.
- On-call rotation every 11-14 weeks, with 2 weekday nights per month and 1 Holiday rotation per year.
