About The Position

St. Croix Hospice seeks a Registered Nurse Case Manager to provide individualized, holistic support to patients in their homes. The role collaborates with the care team to develop personalized plans of care, leads end-of-life conversations, and supports symptom management with empathy and clinical excellence. The position is based in Mount Pleasant, Wisconsin, includes 100% travel, and has an 8:00 a.m. to 5:00 p.m. schedule with on-call expectations.

Requirements

• Associate, bachelor's, or master's degree from an accredited nursing program and Registered Nurse (RN) qualification.
• At least 1 year of recent acute care experience.
• Reliable transportation, a valid driver's license, and proof of auto insurance.

Nice To Haves

• Hospice or home health experience.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patients and families, updating them as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
