About The Position
St. Croix Hospice is seeking a Registered Nurse Case Manager to provide individualized, holistic support to patients in their homes. The role is central to patient care and involves collaborating with a care team to develop personalized plans of care, leading end-of-life conversations, and providing empathetic, clinically excellent symptom management. The position is full-time, based in Wentzville, Missouri, with a stated travel percentage of 100% and on-call expectations. The schedule is 8:00 a.m. to 5:00 p.m.
Requirements
- Associate, bachelor's, or master's degree from an accredited nursing program and current Registered Nurse qualification.
- At least 1 year of recent acute care experience.
- Reliable transportation, a valid driver's license, and proof of auto insurance.

- Hospice or home health experience.
Responsibilities
- Complete initial and ongoing comprehensive assessments to determine hospice needs.
- Develop individualized care plans with patients and families, updating them as needs evolve.
- Administer medications and treatments as prescribed.
- Educate patients and caregivers on care techniques, medications, diet, and symptom management.
- Rotate on-call duties and ensure seamless care transitions.
- Collaborate with the care team to develop personalized plans of care.
- Lead end-of-life conversations and provide symptom management.
