About The Position

This role involves architecting comprehensive care plans that bridge the gap between hospital and home, ensuring every step aligns with the patient’s personal values and recovery goals. The nurse will advocate for patients by identifying and removing systemic barriers to care, ensuring they have the essential resources needed for a safe transition. This position also requires orchestrating clinical communication across the multidisciplinary team to ensure the health plan is executed seamlessly and championing documentation integrity by serving as a clinical resource, ensuring medical records accurately reflect the necessity of services and the complexity of the patient's journey.

Requirements

Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
Diploma from an accredited school/college of nursing required.

Nice To Haves

BLS Provider credentialed from the American Heart Association (AHA) preferred.
Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
