About The Position

This role involves orchestrating the overall plan of care from admission to discharge, integrating patient and family preferences into a cohesive strategy across the entire continuum of care. The Registered Nurse will serve as the clinical expert in assessing and treating complex needs, including legal issues, neglect, substance abuse, crisis intervention, and psychosocial adjustment. This position will proactively identify and dismantle barriers to care, advocating for the internal and community resources necessary to ensure patient safety and health equity. A continuous dialogue with the care transition team will be maintained to reevaluate health plans and provide expert guidance on medical record documentation, complex level-of-care recommendations, and post-discharge options.

Requirements

  • Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

  • Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.
