About The Position

Coordinate and manage the overall interdisciplinary plan of care for patients, ensuring clinical needs are met at every stage of their stay. Create comprehensive care plans that span the entire healthcare continuum, actively integrating patient/family preferences, cultural values, and personal goals. Act as an expert resource for identifying and arranging appropriate treatment options and support services for post-discharge care.

Requirements

• Licensed Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
• Licensed as a Registered Nurse in Florida required.
• Credentialed from State Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.
• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
• Previous case management experience

Nice To Haves

• BLS Provider preferred. American Heart Association or American Red Cross accepted.
