About The Position

This role is for a Registered Nurse in Case Management within the Associate Travel Program at Ascension St. Thomas West in Nashville, TN. The position is a day shift with 40 hours per week, offering a gross rate up to $2,590 per week. The nurse will develop and integrate patient/family-centered care plans, continuously assess care through medical record review, secure resources, and remove care barriers. They will also coordinate with supervisors and providers for effective care implementation, manage documentation, verify service appropriateness, and advise on post-discharge treatment.

Requirements

Registered Nurse obtained prior to hire date or job transfer date required.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
1+ year of recent experience in Case Management.

Nice To Haves

BLS Provider preferred. American Heart Association or American Red Cross accepted.
Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
